Project HOOP Performing Arts Needs Assessment Survey

Project HOOP is currently working to bring Native American theater and performing arts into communities across Indian Country. It is our goal at Project HOOP to develop programming, which is tailored to the individual needs of Native communities like yours. We are currently conducting a needs assessment survey in order to ascertain whether your community would be interested in developing Native theater and/or a performing arts related project. Results from this survey will help to facilitate programming such as artist-in-residence programs, sponsoring performing arts workshops, and teacher training within tribal communities.

Participation in this survey is voluntary. Your completed questionnaire will be accessible only to Project HOOP staff. Your responses may appear in reports, which may be shared with the Ford Foundation, possibly other funding organizations, and/or Native American tribes/organizations, but you will remain anonymous for reporting purposes. In addition, the questionnaire asks if we may contact you by telephone. We may call you to ask permission to conduct a short, 10-15 minute telephone interview as part of our survey.
